In this randomized controlled trial, roflumilast does not change markers of arterial stiffness or endothelial dysfunction after administration for 6 months in patients with stable COPD. We observed a significant and clinically important improvement in exercise capacity as a secondary end point.

Skeletal muscle wasting (sarcopenia) without obvious weight loss is commonly found in patients with IPF receiving AFT. Most study patients died of chronic respiratory failure and skeletal muscle loss was associated with worse outcomes in AFT, suggesting that sarcopenia prevention is crucial for patients with IPF. |

Probe based confocal laser endomicroscopy (pCLE) is an optical imaging tool allowing live imaging of tissues at a cellular level. It remains experimental but its clinical value as a diagnostic/guiding tool is apparent. To address the lack of data in thoracic oncology and pleural diseases, we show the ability of pCLE during medical thoracoscopy to distinguish benign from malignant pleural involvement. |

This is the first multicentre prospective registry reporting baseline characteristics and estimated survival of patients with chronic thromboembolic pulmonary hypertension (CTEPH) in China. A total of 593 patients with CTEPH were included. The long-term survival rates are similar to those of patients in the international and Spanish registries. Pulmonary endarterectomy is an independent predictor of survival. |
