We assessed the roles of nocturnal asthma-like symptoms (NAS) and bronchial hyper-reactivity (BHR) in obstructive sleep apnoea (OSA)–asthma association. OSA is associated with NAS with or without the presence of asthma, but BHR is not associated with NAS when asthma is absent. Some NAS perceived as nocturnal asthma could be symptoms of OSA. |

We aimed to determine whether acute chronic obstructive pulmonary disease (COPD) exacerbations may trigger major adverse cardiac events and found an almost fourfold increased risk of myocardial infarction, stroke and cardiovascular death in the period following the exacerbation onset. The risk was particularly high for exacerbations requiring hospitalization and among the oldest individuals.

A prospective, randomized, parallel group study in adult patients with cystic fibrosis (CF) and sleep desaturation to compare 12 months of non-invasive ventilation (NIV) ±O2 to low-flow oxygen therapy (LFO2) during sleep was conducted. NIV ±O2 increases event-free survival over 12 months in adults with CF. No differences were seen in arterial blood gases (ABG), lung function, health-related quality of life (HRQoL) or hospitalizations.

Automated expiratory positive airway pressure (EPAP) titration may be helpful in treating patients with respiratory failure and upper airway obstruction. This study demonstrates that, for a single night's titration, an automated EPAP algorithm is as effective as manually set EPAP for treating desaturation, without compromising ventilation and potentially improving sleep quality. |

We highlight the ability of human rhinovirus (RV) to infect both large and small airway epithelial cells of lung allograft recipients resulting in cell death and inflammation. Importantly, we show that prophylactic treatment with azithromycin predominantly mitigates viral replication and the resulting inflammatory effects of viral infection. |
