Cystic fibrosis modulator therapy can reverse cystic bronchiectasis
Peter G Middleton, Nicholas J Simmonds
DOI: 10.1002/rcr2.1172
Bronchiectasis, previously described as permanent and progressive airway dilatation, ranges from cylindrical to varicose to cystic. This report describes the reversal of cystic bronchiectasis following a year of CF modulator therapy, providing exciting new insights into the pathogenesis of bronchiectasis in CF. |

DOI: 10.1002/rcr2.1173
Persistent air leaks in mechanically ventilated patients can be challenging. Multimodality approach using various bronchoscopic techniques followed by pleurodesis can be game changer |
Endobronchial indocyanine green instillation to identify the intersegmental plane for successful segmentectomy
Paul Lilburn, Jonathan Kwan, Jonathan Williamson, Kevin Ho-Shon, Mohammad Azari, Michael Wilson, Alvin Ing, Tajalli Saghaie
DOI: 10.1002/rcr2.1174
We describe three cases of anatomical segmentectomy (AS) using the combination of endobronchial infiltration of indocyanine green (ICG) to identify the intersegmental plane (critical for the performance of AS), and Computed Tomography (CT) guided methylene blue injection for lesion localisation. The operations were completed successfully demonstrating satisfactory post-operative outcomes including lesion resection with clear surgical margins and acceptable length of stay. We believe that endobronchial instillation of ICG and CT-guided methylene blue injection for lesion localisation show promise as a technique to complement parenchymal sparing thoracic oncological surgery. |

Rare T263P epidermal growth factor receptor extracellular domain mutation of advanced non-small cell lung cancer in a Vietnamese male patient
Kien Hung Do, Tai Van Nguyen, Tu Anh Do, Duc Thanh Le, Phuong Thi Bich Nguyen, Chu Van Nguyen
DOI: 10.1002/rcr2.1170
In this study, we first report a patient with advanced lung adenocarcinoma harbouring only a rare T263P EGFR mutation who benefited from first-line afatinib therapy in Vietnam. The patient achieved a partial response with a time-to-treatment failure of 5 months. The patient subsequently received several chemotherapy regimens as the disease progressed, with overall survival of 17 months. Non-small cell lung cancer with a rare T263P EGFR mutation responds to afatinib but has a poor prognosis. |
Simultaneous regression of non-small cell lung cancer and orbital extranodal marginal zone lymphoma with chemoradiotherapy for lung cancer
Issei Oi, Zentaro Saito, Takanori Ito, Takuma Imakita, Osamu Kanai, Kohei Fujita, Hiromasa Tachibana, Minako Mori, Koki Morioshi, Tadashi Mio
DOI: 10.1002/rcr2.1171
This report describes the case of a 79-year-old Japanese man diagnosed with orbital extranodal marginal zone lymphoma (EMZL) and stage IIIA lung cancer. The patient received concurrent chemoradiation for lung cancer with carboplatin/paclitaxel treatment, resulting in regression of both the lymphoma and lung cancer. |
Mediastinal hematoma following thrombolysis for acute pulmonary thromboembolism
Chitra Veluthat, Uma Maheswari Krishnaswamy, Kavitha Venkatnarayan, Priya Ramachandran, Uma Devaraj
DOI: 10.1002/rcr2.1175
This case report describes a rare complication following thrombolysis for acute massive pulmonary embolism. Our patient developed a mediastinal hematoma following thrombolysis, which was evacuated surgically. |
One-stop-shop: Lung cancer and pheochromocytoma diagnosed by robotic-assisted bronchoscopy and EBUS scope on a single endoscopic event
Anoop Koratala, Nikitha C Chandra, Alejandra Yu Lee-Mateus, David Abia-Trujillo, Britney N Hazelett, Janani Reisenauer, Ryan Chadha, Sebastian Fernandez-Bussy
DOI: 10.1002/rcr2.1176
Here we present a case of lung and adrenal unrelated malignancies diagnosed by shape-sensing robotic-assisted bronchoscopy (ssRAB) and EBUS-guided biopsy, respectively, and mediastinal staging within the same endoscopic procedure. |
Pulmonary hypertension secondary to interstitial fibrosis with pulmonary venous lesions masquerading pulmonary veno-occlusive disease
Hiroki Nakayama, Masayuki Ishida, Hiroshi Nakaoka, Yuki Nishimura, Ryu-ichirou Imai, Hiroki Sugane, Hayato Hosoda, Yoko Nakaoka, Koji Nishida, Shu-ichi Seki, Sho-ichi Kubokawa, Kazuya Kawai, Naohisa Hamashige, Yoshinori Doi
DOI: 10.1002/rcr2.1179
Clinical presentation of pulmonary hypertension due to interstitial fibrosis with pulmonary venous lesions may simulate those of pulmonary veno-occlusive disease. Cautious diagnostic and therapeutic approaches are required. |
Mixed Mycobacterium kansasii and Mycobacterium smegmatis infection in an adult-onset immunodeficiency patient with anti-interferon-γ autoantibodies
Yun-Tse Chou, Wei-An Liao, Chin-Wei Kuo
DOI: 10.1002/rcr2.1178
In this case report, we present a newly diagnosed AIGA patient who initially presented with disseminated mycobacterial infection. PCR-based testing revealed a mixed pulmonary infection with Mycobacterium kansasii and Mycobacterium smegmatis, as well as disseminated M. kansasii infection. After completing 12 months of antibiotics treatment, the infection resolved, and there was no evidence of reactivation, even in the absence of AIGA-specific treatment. |

Primary pulmonary lymphoma diagnosed by ultrasound-guided transthoracic needle biopsy
Yasuhito Sekimoto, Mitsuaki Sekiya, Makiko Kohmaru, Tomoko Okuma, Manabu Tajima, Hideaki Sato, Kazuhisa Takahashi
DOI: 10.1002/rcr2.1159
Ultrasound-guided transthoracic needle biopsy is a relatively safe procedure diagnosing subpleural pulmonary mass and has high sensitivity in the diagnosis of lung cancer. However, the usefulness in other rare malignancies is unknown. This case shows the effectiveness in diagnosing not only lung cancer but also rare malignancies including primary pulmonary lymphoma. |

DOI: 10.1002/rcr2.1183
Diffuse pulmonary meningotheliomatosis (DPM) is a rare pulmonary disease of unknown aetiology. These imaging findings are characteristic of DPM, adding to the limited number of literature reports on the rare disease. |
Paediatric foreign body aspiration and endobronchial granuloma
Adeline Y L Lim, Sadasivam Suresh
DOI: 10.1002/rcr2.1184
Paediatric foreign body aspiration may result in pulmonary complications. Both surgical and bronchoscopic intervention have been described in managing larger endobronchial lesions. In this case, complete resolution of the granulomatous lesions was achieved with conservative management. |
