Amiodarone-related interstitial lung disease may exhibit variable clinical, radiographic, and histologic presentations. Prompt discontinuation and avoiding re-exposure of the agent is important to prevent the progression and recurrence of the interstitial lung disease.

Massive pulmonary embolism led to cardiac arrest two days after thoracoscopy in a young male with pleural tuberculosis

We describe a fatal case of a 30-year-old previously healthy male with pleural tuberculosis who developed a massive pulmonary embolism with subsequent cardiac arrest after a diagnostic medical thoracoscopy. Computed tomography pulmonary angiogram showed major right pulmonary embolism. Unfortunately, the patient passed away despite resuscitation and extensive organ support in the intensive care unit. This case highlights the thrombotic risk in this population group in order to avoid such devastating complications.

Ultrasound-controlled transthoracic true-cut needle biopsy in pulmonary nodular amyloidosis

The current case report presents a 59-year-old man with imaging studies of the thorax showing nodular lesions in the lungs bilaterally. Preliminary diagnoses for possible granulomatosis (tuberculosis) or pulmonary metastatic dissemination of a neoplastic process were made. An ultrasound-controlled transthoracic true-cut needle biopsy of a subpleural lesion was performed, confirming the diagnosis of ‘pulmonary nodular amyloidosis’.

Combined treatment with beaded EWS, Gore-tex patch and N-butyl-2-cyanoacrylate for bronchial fistula: The novel “sandwich method”

In this report, we describe, for the first time, a novel method for closing bronchopleural fistula (BPF) using an endobronchial Watanabe spigot (EWS), polyglycolic acid (PGA) sheet and N-butyl-2-cyanoacrylate (NBCA). We named this method the “sandwich method.”

Intrapleural therapy for pleural infection from bronchopleural fistula in an adult with hyper-IgE syndrome

An adult with hyper-IgE syndrome presented with a large hydropneumothorax secondary to bronchopleural fistula and pleural infection. Alongside standard care, he received two doses of low-dose intrapleural therapy (1 mg tissue plasminogen activator and 5 mg deoxyribonuclease), which allowed complete evacuation of his residual loculated pleural fluid, aided resolution of his infection without provoking a significant air leak and avoided the need for surgery.

Severe asthma remaining well-controlled after mepolizumab discontinuation: A case report and literature review

Mepolizumab, a humanized anti-IL-5 monoclonal antibody used for severe asthma, results in reduced asthma exacerbations, improved lung function, reduced oral corticosteroid use, and improved quality of life. But little is known about the effects of withdrawal after long-term treatment. Here, we report a case of a patient with asthma, well-controlled without exacerbation after withdrawal of mepolizumab.

Pulmonary alveolar proteinosis (PAP) affecting transplanted lungs is not well recognized. Herein, we report two cases of PAP after lung transplantation (LTx) and their management. Transplant physicians should be aware of this rare complication to avoid misconducting immunosuppressive management.

We present a case report of a 92-year-old patient with thoracic empyema, who was successfully treated via CT-guided insertion of a pigtail catheter. We believe that this educational case serves as a testament to the fact that even the most aged patients can be successfully treated with resourcefulness.

Alveolar-pleural and bronchopleural fistulas that do not improve with thoracic drainage in patients who are unfit for surgery can be successfully managed by endobronchial occlusion and pleurodesis. However, the strategy to be followed in case of failure in conventional conservative management is unclear. We report a case of alveolar-pleural fistula managed by bronchial occlusion using a combination of Endobronchial Watanabe Spigot and N-butyl-2-cyanoacrylate.

A hole lot of trouble: Severe bilateral anastomotic dehiscence following bilateral single sequential lung transplant

Rapid onset of effect of benralizumab in a severe eosinophilic and allergic asthma patient with allergic bronchopulmonary aspergillosis

Case report of a 65-year-old woman with combined severe eosinophilic and allergic asthma, who was later diagnosed with concomitant allergic bronchopulmonary aspergillosis. Benralizumab demonstrated a significant role in reducing the exacerbation rate and oral corticosteroid use in this patient, as well as improving lung function, asthma control, and quality of life measures.

Azygos vein thrombosis (AVT) is an extremely rare type of venous thromboembolism. It is commonly associated with predisposing factors such as azygous vein aneurysm and malignancy including cholangiocarcinoma and renal cell carcinoma. Here we report a rare case of AVT associated with metastatic breast cancer.

Furious lung abscess due to Parvimonas micra

We present a case of lung abscess due to Parvimonas micra in a 57 year old woman who did not respond to antibiotics and needed surgical resection, indicating the high virulence of this bacteria.

Intrapleural findings of pulmonary light-chain deposition disease

Light-chain deposition disease is accompanied not only by characteristic CT findings but also by characteristic gross findings of the visceral pleura. Medical thoracoscopy could diagnose LCDD by the findings of the thoracic cavity.
