Two cases of cryptogenic organizing pneumonia masquerading as tuberculosis (TB) in a TB endemic area

We report two patients with cryptogenic organizing pneumonia (OP) mimicking tuberculosis (TB). These cases highlight the rare presentation of OP and serves as a reminder that patients tested negative for TB, despite typical history and chest imaging findings, warrant further investigations as many diseases may mimic TB and vice versa. |

We report a case with suspected expiratory central airway collapse (ECAC), where initial workup with standard forced expiratory computed tomography (CT) could not detect expiratory collapse. Given the high index of suspicion for ECAC, the patient underwent dynamic bronchoscopy confirming the diagnosis. Subsequently, the implementation of a novel CT protocol, dynamic forced expiratory (cinematic), which comprises detailed, consecutive helical imaging of the central airway throughout the entire respiratory cycle could also confirm severe ECAC as proven by dynamic bronchoscopy. |

Inflammatory myofibroblastic tumour of the lung is a rarely encountered lesion. Although it is commonly seen in children, the number of childhood cases in the literature is limited. It has a dual nature: it may behave as an inflammatory lesion or a tumour with the capacity for recidivism and metastasis. So, the syndrome requires careful follow-up. |

Hyperimmunoglobulin E (IgE) syndrome (HIES) is rare disease characterized by recurrent infections, eczema and increased serum level of IgE. Chronic pulmonary aspergillosis (CPA) is a chronic progressive infectious disease common in patients with underlying pulmonary diseases, but there are few reports of complications due to HIES. We report a case of HIES comorbid with CPA in a teenage male. |

Thymomas are often associated with paraneoplastic disorders. Patients with thymoma presenting with opportunistic infections should be screened for hypogammaglobulinaemia. We describe a case of Good's syndrome presenting with cytomegalovirus pneumonitis and oesophageal candidiasis, and an incidental diagnosis of renal cell carcinoma. |
A case of effective intravenous methylprednisolone pulse therapy against severe COVID-19 infection after arteriovenous graft surgery

COVID-19 infections in patients suffering from end-stage renal disease (ESRD) are more likely to become severe than in other patients, with a high mortality rate. Among the therapeutic agents known to be effective, only corticosteroids can be used in patients suffering from ESRD, significantly limiting treatment options. We report a case of ESRD in which COVID-19 infection immediately followed arteriovenous graft surgery. Although the respiratory condition deteriorated following dexamethasone administration, requiring invasive mechanical ventilation, pulse therapy was successful. |

A case of transbronchial lung cryobiopsy (TBLC) performed on a lung lesion in a patient with adult T-cell lymphoma/leukaemia (ATLL) diagnosed with Pneumocystis jirovecii pneumonia is presented. This is an important report that shows the usefulness of TBLC in differentiating pulmonary lesions in ATLL. |
Association of COVID-19 antigenicity with the development of antineutrophilic cytoplasmic antibody vasculitis

We present two cases of antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis (AAV): one that developed after coronavirus disease 2019 (COVID-19) infection presenting as diffuse alveolar haemorrhage and another that developed shortly after vaccination, presenting as granulomatous pulmonary nodules. Both patients improved with supportive care and immunosuppressive therapies. This adverse event appears to be a very rare complication of COVID-19 infection or vaccination. Early diagnosis of AAV is important because immunosuppressive therapy may improve patient outcomes. |

Urachal cancer, a rare malignancy, constitutes <1% of all bladder cancers. The prognosis of metastatic urachal cancer is extremely poor. Cases of distant metastasis without local recurrence alone are relatively rare. We describe a case of multiple metastatic lung cancer from urachal carcinoma that required differentiation from primary lung cancer. |
