Topical mitomycin-C as an adjuvant to multimodal endoscopic treatment for tracheobronchial stenosis secondary to endobronchial tuberculosis

Tracheobronchial stenosis secondary to endobronchial tuberculosis is a debilitating disease but its optimal treatment strategy remains undefined. With a good safety profile, topical mitomycin-C (TMC) appears to be a useful adjuvant to endoscopic treatment for tuberculosis-related airway stenosis. This case series aims to highlight the current gaps in knowledge regarding the optimal role of TMC and whether certain airway features such as bronchomalacia or thick fibrotic band can influence its efficacy. |
Imaging for precision medicine: can V-P SPECT measure mepolizumab response in asthma?

We report a case series describing treatment response to mepolizumab in four severe asthma patients, assessed by traditional methods and with objective ventilation/perfusion single photon emission computed tomography (V-P SPECT). In this series, patients with severe asthma received mepolizumab treatment with clinical outcomes recorded and V-P SPECT imaging performed at commencement and at approximately 16 weeks post-treatment initiation. V-P SPECT shows promise as an objective measure to assess lung ventilation and responsiveness to mepolizumab, allowing better precision in determining treatment improvements. |

Diagnosis of unexposed tumours using endobronchial ultrasonography with a guide sheath and a thin transbronchial needle

We report two cases of unexposed tumours diagnosed successfully using transbronchial biopsy (TBB) done by placing a guide sheath with a thin transbronchial needle into the tumours . Case 1 presents a typical carcinoid in the peripheral lung. Case 2 presents a squamous cell carcinoma at the third bifurcation of the right lung. TBB samples obtained this way were larger than transbronchial needle aspiration (TBNA) samples. Moreover, multiple TBBs were possible once the guide sheath was inserted intratumorally. This novel technique will make a significant contribution towards diagnosing unexposed lung tumours. |

We report a case of multidrug-resistant M. simiae respiratory infection in a 65-year-old woman with a history of previous Mycobacterium tuberculosis infection. |

We observed the development of refractory spontaneous pneumothorax during nintedanib therapy in two patients with systemic sclerosis and interstitial lung disease (SSc-ILD) and impaired lung function. Nintedanib use for SSc-ILD, an extensive disease, may therefore increase the risk of pneumothorax. In addition, pneumothorax is more likely to be refractory in these cases; initiation of nintedanib treatment and follow-up should be considered carefully. |
