Topical mitomycin-C as an adjuvant to multimodal endoscopic treatment for tracheobronchial stenosis secondary to endobronchial tuberculosis
Nai-Chien Huan, Khai Lip Ng, Mona Zaria Nasaruddin, Noorul Afidza Muhammad, Ummi Nadira Daut, Jamalul Azizi Abdul Rahaman
DOI: 10.1002/rcr2.711

Tracheobronchial stenosis secondary to endobronchial tuberculosis is a debilitating disease but its optimal treatment strategy remains undefined. With a good safety profile, topical mitomycin-C (TMC) appears to be a useful adjuvant to endoscopic treatment for tuberculosis-related airway stenosis. This case series aims to highlight the current gaps in knowledge regarding the optimal role of TMC and whether certain airway features such as bronchomalacia or thick fibrotic band can influence its efficacy.

Imaging for precision medicine: can V-P SPECT measure mepolizumab response in asthma?
Vanessa M McDonald, Paola D Urroz, Marika Bajc, Natalie Rutherford, Bree Brooker, Peter G Gibson
DOI: 10.1002/rcr2.717

We report a case series describing treatment response to mepolizumab in four severe asthma patients, assessed by traditional methods and with objective ventilation/perfusion single photon emission computed tomography (V-P SPECT). In this series, patients with severe asthma received mepolizumab treatment with clinical outcomes recorded and V-P SPECT imaging performed at commencement and at approximately 16 weeks post-treatment initiation. V-P SPECT shows promise as an objective measure to assess lung ventilation and responsiveness to mepolizumab, allowing better precision in determining treatment improvements.

Hadi Lotfi, Mojtaba Sankian, Zahra Meshkat, Ahmad Khalifeh Soltani, Ehsan Aryan
DOI: 10.1002/rcr2.719

In the current study, we surveyed the clinical features, risk factors, diagnosis, and treatment of Mycobacterium simiae pulmonary infection in Mashhad, Iran.

Case Reports
Spindle cell sarcoma: a rare cause of a spontaneous pneumothorax
Ana Pedroso, Lutz Beckert
DOI: 10.1002/rcr2.709

We present the case of an asymptomatic spontaneous pneumothorax which led to the diagnosis of a primary spindle cell sarcoma of the pleural. We review the literature and highlight the peculiarity of this case.

Unusual presentation of recurrent follicular lymphoma as diffuse granular shadow
Takayasu Ito, Shotaro Okachi, Yuichi Ishikawa, Satoko Shimada, Keiko Wakahara, Naozumi Hashimoto
DOI: 10.1002/rcr2.710

We herein describe a case of pulmonary lesion presenting with diffuse granular shadow that was diagnosed as recurrent follicular lymphoma.

The reverse halo sign and spontaneous pneumothorax due to pulmonary mucormycosis
Muhammad Umair Khawar, Maida Tiwana, Ruchira Sengupta, Jiang Wang, Veronica Indihar
DOI: 10.1002/rcr2.712

We present the unfortunate case of a young female who presented with an atypical radiographic presentation of invasive pulmonary mucormycosis in the form of necrotizing cystic changes leading to spontaneous pneumothorax in addition to reverse halo sign on chest imaging.

Diagnosis of unexposed tumours using endobronchial ultrasonography with a guide sheath and a thin transbronchial needle
Haruka Kuno, Rei Sainouchi, Takayuki Simamoto, Aya Miyagawa-Hayashino, Yoshizumi Takemura
DOI: 10.1002/rcr2.713

We report two cases of unexposed tumours diagnosed successfully using transbronchial biopsy (TBB) done by placing a guide sheath with a thin transbronchial needle into the tumours . Case 1 presents a typical carcinoid in the peripheral lung. Case 2 presents a squamous cell carcinoma at the third bifurcation of the right lung. TBB samples obtained this way were larger than transbronchial needle aspiration (TBNA) samples. Moreover, multiple TBBs were possible once the guide sheath was inserted intratumorally. This novel technique will make a significant contribution towards diagnosing unexposed lung tumours.

A case of diffuse large B-cell lymphoma originating from chest wall complicated by benign asbestos pleural effusion
Nobuyuki Kondo, Yukihisa Inoue, Hiroaki Takeyama, Akiko Kobayashi, Osamu Matsubara, Yasuto Jinn
DOI: 10.1002/rcr2.714

Here, we report a case of primary diffuse large B-cell lymphoma (DLBCL) presenting as chest wall tumour complicated by benign asbestos pleural effusion, which was confirmed by autopsy.

A case of multidrug-resistant Mycobacterium simiae in an elderly woman
Hadi Lotfi, Ehsan Aryan, Mojtaba Sankian, Zahra Meshkat, Ahmad Khalifeh Soltani, Amir Hooshang Alvandi, Hadi Farsiani
DOI: 10.1002/rcr2.715

Mycobacterium simiae is an emerging and spreading pathogen in Iran and little data about its drug susceptibility test (DST) and no standard treatment regimen are available. We report a case of multidrug-resistant M. simiae respiratory infection in a 65-year-old woman with a history of previous Mycobacterium tuberculosis infection.

Spontaneous pneumothorax during nintedanib therapy in patients with systemic sclerosis-associated interstitial lung disease
Toshiyuki Sumi, Hirofumi Uehara, Makoto Tada, Yoshiko Keira, Koki Kamada, Naoki Shijubou, Yuichi Yamada, Hisashi Nakata, Yuji Mori, Hirofumi Chiba
DOI: 10.1002/rcr2.716

We observed the development of refractory spontaneous pneumothorax during nintedanib therapy in two patients with systemic sclerosis and interstitial lung disease (SSc-ILD) and impaired lung function. Nintedanib use for SSc-ILD, an extensive disease, may therefore increase the risk of pneumothorax. In addition, pneumothorax is more likely to be refractory in these cases; initiation of nintedanib treatment and follow-up should be considered carefully.

Pneumothorax with Ortner syndrome: an unusual presentation of aortic dissection
Shan Min Lo, Hema Yamini Ramarmuty, Kunji Kannan
DOI: 10.1002/rcr2.718

This study describes pneumothorax with Ortner syndrome, which is an unusual presentation of aortic dissection.
