Mediastinal cysts with Mullerian differentiation

Mediastinal cyst with Mullerian differentiation is characterized by estrogen receptor and progesterone receptor positive cells lining. The tumors were located in the para-vertebral areas around the Th-1 to 7 vertebrae.

We present a case of tracheal adenoid cystic carcinoma (ACC), a rare type of primary airway tumour, diagnosed in a pregnant lady. We also discuss the characteristics and treatment of 13 other cases of primary tracheal tumours reported during pregnancy, 2 cases of which were ACC.

The case report describes the complication of bronchopleural fistula after a cryoprobe biospy. The patient was successfully treated with a one-way endobronchial valve.

Successful ventilator weaning following vascular bypass in patient with vascular tracheobronchial compression

Benign tracheal stenosis is most commonly caused by congenital vascular anomalies. A much rarer cause is termed vascular tracheobronchial compression syndrome, which primarily occurs in adults. Here we describe a previously unreported pattern involving tracheal compression between the right aortic arch and the left common carotid artery, which occurred in a patient who had undergone thoracoplasty for tuberculous sequelae 54 years earlier. Vascular bypass surgery enabled successful ventilator weaning.

Endotracheal tube clamping and extracorporeal membrane oxygenation to resuscitate massive pulmonary haemorrhage

Massive pulmonary haemorrhage is a life-threatening and difficult-to-manage condition. In certain circumstances, traditional approaches for haemoptysis may not work well. We report a case of life-threatening pulmonary haemorrhage which was successfully controlled by airway tamponade therapy with endotracheal tube clamping under the support of extracorporeal membrane oxygenation (ECMO).

Pseudomonas pharyngitis in a cystic fibrosis patient

We report a case of Pseudomonas pharyngitis in a cystic fibrosis (CF) patient not known to have prior colonisation of Pseudomonas. We believe that this is one of the first cases of pseudomonas pharyngitis in a CF patient.

Non-specific interstitial pneumonia associated with clinically amyopathic dermatomyositis showing "crazy paving" appearance on thin-section lung CT

The "crazy paving" appearance consists of ground-glass opacity superimposed on a network of linear opacities on thin-section computed tomography (CT) images of the lung. This finding has been described in a variety of diseases but is extremely rare in patients with non-specific interstitial pneumonia (NSIP). We describe a 45-year-old woman with biopsy-proven NSIP associated with clinically amyopathic dermatomyositis that showed a "crazy paving" appearance on thin-section CT of the lung. Clinicians should include NSIP in the differential diagnosis in patients presenting with "crazy paving" appearance on thin-section chest CT.

Diffuse endobronchial metastasis from sigmoid carcinoma presenting as wheezing and respiratory failure

This case highlights a rare presentation of endobronchial metastasis from sigmoid carcinoma with persistent wheezing, respiratory failure, and difficulty in weaning from mechanical ventilation.
