Mepolizumab asthma treatment failure due to refractory airway eosinophilia, which responded to benralizumab

Monoclonal antibodies directed against interleukin (IL)‐5, such as mepolizumab and benralizumab, are an effective and established treatment for severe eosinophilic asthma. Here, we present a patient with eosinophilic asthma with a partial clinical response to mepolizumab, although with refractory airway eosinophilia, who did not fully respond to dose escalation. A treatment trial with benralizumab was more successful and led to suppression of airway eosinophilia. |

Pulmonary fibrosis after coronavirus disease 2019 (COVID‐19) recovery is becoming an emerging threat to public health worldwide. Given the present case, nintedanib might provide a novel therapeutic approach for managing post‐COVID‐19 fibrosis, although further studies are warranted. |

We report a patient previously diagnosed with adult‐onset still's disease (AOSD) who presented with pulmonary artery hypertension (PAH) but without typical symptoms of AOSD. Immunosuppressive agents rapidly improved PAH. |

We report a case of vascular Ehlers–Danlos syndrome (EDS) diagnosed in a patient as cystic lung disease with recurrent pneumothorax. This case highlights the importance of detailed history‐taking, physical examination, and clinical suspicion of vascular EDS when encountering recurrent pneumothorax cases refractory to conventional therapy. |

One delayed diagnosis of paragonimiasis case and literature review

Human paragonimiasis has been appearing all over the world due to increased human migration, international travel, and worldwide food trading. However, delayed and missed diagnosis rates are also increasing due to atypical clinical manifestations and the lack of disease understanding by clinical workers. Here, we present a case of delayed diagnosis in a 43‐year‐old male. The key findings for correct diagnosis were patient's living in mountainous areas with streams with the presence of host, hypereosinophilia, and careful diet history. Once diagnosed, praziquantel is preferred for treatment. |

The primary treatment is often radical resection; however, we effectively used endobronchial resection, a less invasive procedure to manage the airway using a flexible and rigid bronchoscope, in our patient. |

When a chest computed tomography (CT) scan is performed in the diagnosis and treatment of coronavirus disease 2019 (COVID‐19) pneumonia, the possibility of lung neoplasm should be kept in mind if the ground‐glass nodule (GGN) shows characteristics that are non‐specific for viral infection, such as solitary nature, relative roundness, well‐defined borders, and distance from the pleura. |
