Paradoxical cause of weaning failure: Post tracheostomy foreign body causing non-resolving pneumonia with worsening failure to wean

We describe a case of broken tracheal cartilage which slipped into the tracheobronchial tree following percutaneous tracheostomy (PCT), impinging and causing collapse of medial basal segment of right lower lobe. Our case highlights a rare complication of PDT leading to weaning failure, irony of situation being that it was done to treat the same. |

The coronavirus disease-2019 (COVID-19), an emerging infectious disease caused by Severe Acute Respiratory Syndrome-Coronavirus 2 (SARS-CoV2), emerged in December 2019 and has spread worldwide. Old age, hypertension, diabetes, and smoking were identified risk factors for aggravation. Obesity also increases the severity and mortality of COVID-19. However, the relationship between asthma and COVID-19 infection remains unclear. We present a rare case of an obese asthmatic patient who experienced asthma exacerbation during COVID-19 infection. |

Chronic bronchiectasis may be complicated with non-ischaemic and non-infectious pneumatosis intestinalis. Clinicians managing patients with bronchiectasis should be alert to such an unusual extrapulmonary complication, because either neglecting the condition or unnecessary exploratory surgery may lead to hazardous outcomes. |
Rare case of cystic anterior mediastinal tuberculosis in an immunocompetent patient

We report a rare case of a 28-year-old immunocompetent woman found to have a cystic superior anterior mediastinal mass. She underwent surgical resection of the mass and, based on histological appearances of necrotizing granulomatous inflammation and a Gene Xpert™ positive for Mycobacterium tuberculosis complex, she was subsequently commenced on anti-tuberculous therapy. |

This study reports the case of a patient with combined large-cell neuroendocrine carcinoma (LCNEC) whose symptoms were due to brain metastasis. Because the histological findings of the resected brain tumour and transbronchial biopsy specimen suggested combined LCNEC with a mixture of adenocarcinoma and sarcomatoid components, atezolizumab in combination with carboplatin and nanoparticle albumin-bound (nab)-paclitaxel was selected as the first-line treatment regimen. This treatment resulted in partial response and remained durable for 12 months with an ongoing regimen. |

Narrow band imaging (NBI; Olympus Medical Systems, Tokyo, Japan) has been reported to be useful for bronchoscopic diagnosis of endobronchial lesions, particularly for assessing the vascular characteristics of lesions. ‘Light blue crest’ (LBC) observed using NBI with magnifying endoscopy has been reported in the field of gastrointestinal endoscopy. We experienced a case of endobronchial hamartoma in which a light blue line similar to LBC was observed on the surface using NBI under saline injection. |

We report a very rare case of combined small cell lung carcinoma (C-SCLC) which presented as persistent cough and was due to endotracheal metastases. Clinicians should be aware of this unusual site of metastases from a C-SCLC as recognition and treatment improved our patient condition. |
