Clinical characteristics of anti-MDA5 antibody-positive interstitial lung disease

Patients with anti-melanoma differentiation-associated gene 5 (MDA5) antibody-positive clinically amyopathic dermatomyositis (CADM) usually present with respiratory symptoms with rapid deterioration or rapidly progressive interstitial lung disease (RP-ILD). As RP-ILD has a high mortality rate, an early diagnosis is important to initiate treatment in these patients.

Immunoglobulin G4 (IgG4)-related disease is a systemic inflammatory disorder that can affect almost any tissue. Isolated IgG4 pleural disease is a rare manifestation, and has only been described in patients presenting with dyspnoea. We present a case of asymptomatic isolated IgG4 pleural effusion.

Coronavirus disease (COVID-19) may be associated with multiple organ failure due to vascular endothelial damage. Here, we report a case of COVID-19 in which a patient suffered from ileus complications and was difficult to manage systemically. However, he recovered with high doses of steroids, hemodialysis, and the nasal intestine without the use of antivirals.

Guide sheath breakage during endobronchial ultrasonography

The radiopaque band attached to the head of the guide sheath dislodged and remained in the peripheral bronchus near the tumour during endobronchial ultrasonography with a guide sheath (EBUS-GS). The band could not be removed endoscopically. We performed a partial lung resection to remove the radiopaque band and the nodule together four months after EBUS-GS.

Scedosporium apiospermum lung disease in a patient with nontuberculous mycobacteria

Although tuberculosis has been identified as a major underlying cause of pulmonary mycetoma due to Scedosporium apiospermum infection, little is known about coinfection with nontuberculous mycobacteria (NTM) and S. apiospermum. We now report a case of S. apiospermum lung disease in a patient with NTM infection, but with neither tuberculosis nor an immunosuppressed condition. This case reinforces the fact that clinicians should be aware of the possibility of S. apiospermum infection even in patients who are immunocompetent or free from tuberculosis.

Unique clinicopathological characteristics of pulmonary squamous cell carcinoma with part-solid nodule

We recently experienced a rare case of pulmonary squamous cell carcinoma showing part-solid nodule finding on high-resolution computed tomography. Here, we present the clinicopathological details of the case along with a literature review.

Acute exacerbation in chronic bird fancier's lung with pleuroparenchymal fibroelastosis

This is a case of chronic bird fancier's lung (BFL) (java sparrow and budgerigars) presenting with acute exacerbation due to exposure to feather duvet. Chronic BFL occasionally can associate with pleuroparenchymal fibroelastosis (PPFE). Searching for precipitins against antigens of bird droppings and feather is useful for our clinicians to make a diagnosis of BFL.

Hyaluronic acid in viscous malignant mesothelioma pleural effusion

We report two cases of extraordinarily viscous malignant pleural effusion (MPE) in mesothelioma patients. Our ex vivo experiments attributed the viscosity to the high hyaluronic acid (HA) content within the fluid, which could be significantly reduced with hyaluronidase.

Traumatic iatrogenic chylothorax after external electrical cardioversion

This case report refers to a patient with rapid recurrent episodes of bilateral pleural chylothorax due to damage of the thoracic duct after external electrical cardioversion (EEC) treatment. The diagnosis took place by the method of exclusion, when all known causes of chylothorax, both non-traumatic and traumatic, were ruled out. To the authors' knowledge, no case of traumatic chylothorax after EEC has been reported in the literature.

Mepolizumab use in cystic fibrosis-associated allergic bronchopulmonary aspergillosis

We report a case of successful use of mepolizumab in an adult with cystic fibrosis (CF)-related allergic bronchopulmonary aspergillosis (ABPA), whose treatment was previously complicated by difficulty in steroid wean and drug-related toxicity. Addition of mepolizumab as adjunctive therapy led to clinical stability, facilitated steroid taper, and was well tolerated. To our knowledge, this is the first report of successful mepolizumab treatment of ABPA in CF.

Secondary pulmonary hypertension due to pulmonary Langerhans cell histiocytosis accompanied with panhypopituitarism

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes the coronavirus disease 2019 (COVID-19), has resulted in significant morbidity and mortality worldwide for the past half year. Approximately 5% of COVID-19 patients who suffer from pneumonia develop critical respiratory failure. We reported the case of a healthy 52-year-old man who had respiratory failure owing to SARS-CoV-2 infection and required femoro-femoral veno-venous extracorporeal membrane oxygenation (VV-ECMO) and prone position ventilation (PPV) without any additional complication.
