A case of primary lung cancer requiring differentiation from metastatic cervical cancer

Herein, we report the case of an 80-year-old woman with a history of papillary squamous cell carcinoma of the uterine cervix, presenting with a left pulmonary tumour. A bronchoscopic biopsy revealed squamous cell carcinoma with a papillary pattern, which did not rule out pulmonary metastasis from the cervix. Immunohistochemical staining revealed that the cervical tumour was positive for p16, whereas the pulmonary tumour was negative and was effectively diagnosed as primary pulmonary carcinoma. |

Humidifier lung is a phenotype of hypersensitive pneumonitis (HP) caused by inhalation exposure to contaminated humidifier vapours that frequently occur during dry winter. We report a case of humidifier lung that was diagnosed based on its characteristic clinical cause and detailed medical history taking. |
Extremely late‐onset chronic graft‐versus‐host disease presenting with bronchiolitis obliterans and pleuroparenchymal fibroelastosis

We present the case of 35-year-old patient, with a history of haematopoietic stem cell transplantation (HSCT) at 2 years and 3 months of age during the second disease remission of acute myeloid leukaemia, who was diagnosed with chronic graft-versus-host disease (cGVHD) presenting with bronchiolitis obliterans (BO) and pleuroparenchymal fibroelastosis (PPFE). To our knowledge, this is the first reported case of BO and PPFE diagnosed more than 30 years after HSCT. |
Use of carefully titrated ultra‐low doses of alteplase in infected haemothorax caused by initial alteplase use to drain loculated malignant pleural effusion

This case highlights the safe use of very low doses of alteplase ranging from 0.25 to 0.5 mg following pleural bleeding after the use of alteplase to treat a patient with symptomatic malignant loculated effusion and may demonstrate there may be a use for carefully titrated alteplase in the pleural space after pleural bleeding has stopped to treat pleural infection. |
Atypical distal tracheal fibrous bridge and bronchial stenosis in an adult patient with bronchopulmonary dysplasia

An adult patient with a history of bronchopulmonary dysplasia presented symptoms of tracheobronchial stenosis. We found an unusual tracheal fibrotic bridge connecting the lateral walls of the distal trachea, with adjacent fibrotic stenosis in the left and right mainstem bronchi, treated with an electrocautery knife and balloon dilation without complications. |
Metachronous bilateral immunoglobulin G4‐related pleuritis: A case report and literature review

Here, we describe a case of metachronous bilateral IgG4-related pleuritis. We suspected IgG4-positive plasma cell infiltration from the right to left side, based on both the imaging and pathological findings. The patient also had a complicated postoperative chylothorax on the left side. Although IgG4-RD is systemic, no metachronous bilateral IgG4-associated pleuritis has surprisingly been reported; thus, this case could be key to understanding the pathology of IgG4-RD from a surgical viewpoint. We also review previous reports on the cause of metachronous bilateral pleuritis and postoperative chylothorax. |

Over 150 non-tuberculous mycobacteria (NTM) species have been identified, and the number of NTM infections in Japan has rapidly increased in the past decade. Although NTM infections are rarely observed in patients with cyanotic congenital heart disease, the current trend may increase the incidence of NTM in these patients. Here, we report a case of Mycobacteroides abscessus subsp. abscessus infection in a patient with tetralogy of Fallot. |
