Widespread pulmonary invasion by malignant pleural mesothelioma: an important diagnostic consideration

We report a rare case of early and extensive pulmonary invasion of malignant pleural mesothelioma (MPM) first presenting with hydropneumothorax. She developed widespread and rapidly progressive FDG-avid pulmonary infiltrates. Bronchoalveolar lavage (BAL) and sputum contained mesothelioma cells. The patient deteriorated rapidly with progressive mediastinal and contralateral MPM involvement and died one month later. This case highlights the importance of including tumour invasion as a differential diagnosis of non-resolving pulmonary infiltrates in patients with MPM. |

Organizing pneumonia associated with T-cell lymphoma

We report a rare case of organizing pneumonia (OP) associated with T-cell lymphoma without prior anticancer therapy. OP could represent a syndrome of T-cell dysfunction or develop as a direct reaction to malignant infiltration of the lung. In patients with atypical presentations, exclusion of an alternate diagnosis must be pursued using surgical lung biopsy, wherever possible. |

Paraneoplastic acute fibrinous and organizing pneumonia from lymphoma completely responding to bendamustine-rituximab

Acute fibrinous and organizing pneumonia (AFOP) is a rare histopathological pattern of lung injury characterized by fibrin deposition in alveolar spaces. There is no proven treatment but multiple reports describe response to corticosteroids. We report a compelling instance of paraneoplastic AFOP completely responding to chemotherapy for lymphoma with very limited use of corticosteroids. |

Hot tub lung (HTL) is a pulmonary disease where a hypersensitivity pneumonitis is caused by exposure and inhalation of aerosolized non-tuberculous mycobacteria (NTM) traditionally from hot water steam. We present, to our knowledge, the only case of HTL occurring from an indoor swimming pool affecting five members of a family, in which one member was not a resident of the household. |

We report a case of acute eosinophilic pneumonia (AEP) triggered by coronavirus disease 2019 (COVID-19) infection. This case suggests the importance of considering the complications of AEP when treating patients with COVID-19 infection. |

We report two patients, both with bronchomalacia due to endobronchial tuberculosis (EBTB), who unfortunately developed bronchial tear related to silicone endobronchial stenting. Both patients were successfully managed conservatively without needing emergency open surgery. This case report highlights the current gap in knowledge regarding (1) optimal management and role of silicone airway stents among patients with bronchomalacia secondary to EBTB (B-EBTB) and (2) whether stent-related complication rates are higher among those with B-EBTB. |

We present a case of atezolizumab-induced fulminant type 1 diabetes mellitus (F1DM) that occurred four months after withdrawal of the drug in a 73-year-old Japanese woman highlighting the need for clinicians to closely and continuously monitor the development of immune-related adverse events, including F1DM, irrespective of the number of treatment cycles or the interval after the last immune checkpoint inhibitor cycle. |
